This position focuses on Nursing Facility Transition and is responsible for writing Person Centered Transition Plans and coordinating consumer transition services from the nursing facility to the community and providing monthly support coordination prior to and follow-along services after consumers move back into the community.
Phyllis Reynolds joined the Disability Network Oakland & Macomb team in April 2013. She has studied at Davenport University and University of Phoenix. Phyllis enjoys greeting DNOM’s consumers and guests and ensures that each person’s time spent at DNOM is a positive experience. Phyllis is happy to be part of the DNOM team.
